Every Security Leader understands a breach. Someone gets in. Something gets taken. You contain it, investigate it, report it, and rebuild. That model is no longer sufficient. Some of the highest-impact attacks emerging in AI-driven organizations do not look like breaches. There is no alert, no ransomware note, no obvious indicator of compromise. Nothing is stolen or destroyed in a traditional sense. Instead, the integrity of what your systems rely on begins to shift. Quietly. Gradually. Over time.
By the time the impact is visible, the decisions have already been made. Loans have been approved. Fraud has gone undetected. Risk models have drifted. Outputs that were trusted have been consistently, subtly wrong.
This is data poisoning. And it is one of the least understood, but potentially high-impact, risks in enterprise AI security today.
What data poisoning actually is – At its core, data poisoning is simple.
AI models learn from data. They identify patterns, build associations, and generate outputs based on what they have been exposed to during training or feedback cycles. They do not reason. They reflect. Data poisoning targets that dependency.
An adversary introduces carefully crafted inputs into training pipelines, data lakes, feedback loops, or third-party data sources. Those inputs are blended with legitimate data. The model has no inherent ability to distinguish intent. The result is not immediate failure. In many cases, the model performs normally in testing and validation. It is deployed with confidence. Only later do its decisions begin to diverge in ways that align with the adversary’s objective.
It is important to distinguish this from other AI risks. Data poisoning operates at the level of training and feedback loops, unlike prompt injection or data leakage, which affect runtime behavior or data exposure. While much of the research on data poisoning originates in academic settings, history suggests that the gap between research and operational use can close quickly when incentives are high.

Why financial services and insurance are uniquely exposed
All industries using AI carry risk. Financial services and insurance carry a different category of consequence. In many sectors, a compromised model produces bad outputs. That is serious, but often localized. In financial services, models drive decisions about capital at scale. Credit approvals, fraud detection, underwriting, trading strategies, claims processing. These are not isolated outputs. They are compounding decisions. A small, systematic bias introduced into a model does not result in a single error. It can result in millions of decisions that gradually shift financial exposure, misprice risk, and create regulatory and legal consequences.
Insurance amplifies this further. Actuarial and underwriting models influenced by corrupted data do not fail loudly. They drift. And that drift can affect an entire portfolio over time. An adversary does not need to extract data to create damage. Influencing the quality of decisions can be enough.
The governance gap inside most organizations
In most enterprises, AI adoption has outpaced governance. Tools such as OpenAI’s ChatGPT, Microsoft Copilot, and Google Gemini are already embedded across business functions, often through both approved and unapproved channels. In many organizations, there is no complete inventory of where these tools are being used, what data is flowing through them, or how they are integrated into internal systems.
The more immediate and demonstrable risks today often involve:
- Sensitive data leaving the organization through unmanaged usage
- Lack of visibility into third-party integrations and plugins
- Unclear contractual and technical controls around data handling
Depending on configuration and vendor terms, some platforms may use customer inputs to improve their systems, while enterprise deployments often restrict this behavior. The issue is not that all data is being used for training. The issue is that many organizations have not verified what applies in their environment.
At the same time, every connector, plugin, or integration introduces a new data pathway. Many of these pathways are not centrally governed, audited, or even visible to security teams. This is not just shadow IT. It is a more complex problem where data may be processed, transformed, and potentially influence external systems in ways that are not fully understood.
The framework problem
There is a structural challenge at the center of AI security. Most organizations do not yet have a mature, operational framework for governing AI usage, data flows, and model risk. Standards exist. The NIST AI Risk Management Framework. ISO 42001. Regulatory developments such as the EU AI Act. Policy activity in the United States. But translating these into enforceable controls across a large enterprise environment is non-trivial.
AI risk does not sit cleanly within a single function. It spans security, data governance, legal, compliance, and business operations. Coordination across these groups is often limited, and ownership is unclear. The result is an environment where:
- Accountability is fragmented
- AI tools are widely used
- Data flows are partially understood
- Controls are inconsistently applied
What boards should be asking now
The right question is no longer whether an organization has experienced a traditional breach. It is whether the organization has visibility into how AI systems are influencing decisions, and whether that influence can be trusted. At a minimum, boards should be asking for:
- A comprehensive inventory of AI tools in use across the organization, including unapproved usage
- A clear mapping of what data is being shared with those tools and under what conditions
- A structured assessment of third-party AI vendors, aligned with existing vendor risk management practices
- A current-state evaluation of AI governance, with a defined roadmap and ownership
Organizations that treat AI security as an extension of existing controls without adapting to these dynamics are likely underestimating the risk. Data integrity has always mattered. In AI-driven environments, it becomes foundational.
If the data and feedback loops shaping your models are not understood or controlled, neither are the decisions that depend on them.
